What paperwork is required after the County loan funds have been disbursed?
The business is required to have their new employees complete lncome Verification Forms and the business must complete and submit an annual Job Creation Report. The County will provide the business with these forms to complete. The first form is completed by each new hire. These forms are collected at hiring by the business and pertinent information from these forms is tabulated on the annual Job Creation Report. The business must send all of the original lncome Verification Forms and the Job Creation Report to the Cuyahoga County Department of Development. These forms must be submitted by November 1st so that the County has ample time to review prior to the fiscal year closing. When the business has achieved the job creation figures stipulated in the loan agreement, a staff person the Cuyahoga County Department of Development will visit the business to complete a Final Job Creation Report to closeout that portion of the Loan Agreement.
